The Referral Clerk is responsible to the Site Administrator or on site Nursing Supervisor, and may work closely with patients, nursing staff and providers.
ESSENTIAL DUTIES AND RESPONSIBILITIES: (The following is used as a partial description and is not restrictive as to duties required.)
1. Responsible for the organization and completion of referrals in a timely manner.
2. Maintaining a daily log of all referrals received and enters data into Mega West system, (i.e., demographics, referrals, etc.).
3. Provides daily updates to supervisor regarding referral status.
4. Completion of follow-up on all referrals including documentation in the log, rescheduling appointments and notification of the PCP.
5. Notification and follow-up with the patient and provider specialist of all necessary information for patients appointment.
6. Ongoing knowledge, documentation and update of appropriate referral resources, required forms and key contact persons.
7. Collect and maintain required data for any corporate requirements.
8. May assist patients in filling out paperwork needed for quality care and assist with interpreting as needed.
9. Insures client-patient confidentiality.
10. Other duties and/or special projects as assigned.

Knowledge of:
1. Bilingual Spanish/English/Hmong/Laotian preferred.
Training and Experience:
1. High school graduate or equivalent.
2. Clerical and computer skills to include general knowledge of office procedures and the ability to work with computer systems.
3. Medical office, medical insurance billing or referral experience.
Ability to:
1. Work effectively and professionally with staff, patients, and the public; strong interpersonal/communication skills
2. Handle confidential information.
3. Must be well-organized and detail-oriented, and be able to work in a fast-paced environment.
PHYSICAL REQUIREMENTS:
1. Must be able to lift up to 40 pounds and push up to 100 pounds (on wheels).
2. Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
3. Must have vision with or without lenses adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
4. Must have high manual dexterity.
5. Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
