Position Description: Purpose Responsible for the on-going development, implementation and evaluation of CPSP and other programs providing educational interventions to meet the identified perinatal, newborn and infant health needs of patients served by Golden Valley Health Centers. Duties and Responsibilities • Responsible for the development and updating of protocols and procedures relating to CPSP, perinatal, newborn and infant patient education. • Ensures all programmatic licensing/certification information is up to date. • Responsible for the coordination and integration of the CPSP program components into the patient care setting, ensuring case management is offered and provided to meet patient and family needs. • Work closely with County and State CPSP and Maternal And Child Health program staff in the development of programs that meet programmatic requirements and maximize reimbursement opportunities. • Evaluates work performance of CPSP Patient Care Coordinators; provides support and training to staff and promotes cooperation and team work. • Develop, implement and maintain a lactation support program for GHVC patients and staff. • Completes internal and external reporting requirements as necessary. • Works with department and organizational staff in the development of programs that meet the medical, educational, cultural and linguistic needs of the community and patients GVHC serves. • Participates on organizational and community committees as appropriate. • Completes home and hospital visits to new parents and their infants. • Participates fully in QI activities towards a patient centered care environment. • Carries out supervisory responsibilities in accordance with the organizations policies and applicable laws. Responsibilities include interviewing, training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. • Completes independent projects and/or other duties as requested
Qualifications & Requirements: Min. Qualifications Bilingual English/Spanish encouraged. Willingness to travel within GVHC sites. Own vehicle with valid registration and drivers license; clean DMV printout. Ability to manage sensitive information and maintain confidentiality. Flexibility in working hours. Ability to communicate effectively. Physical Demands The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus. Work Environment The noise level in the work environment is usually quiet. Education/Experience Requirements RN or Bachelors degree in a health related field, with significant work experience in the area of maternal and child health. International Board Certified Lactation Consultant (IBCLC) or a minimum of 80 hours of lactation specific education.
