Overview

Golden Valley Health Centers is excited to introduce a groundbreaking program called Enhanced Care Management (ECM) aimed at providing comprehensive care and support to our valued patients. Through a partnership with the Central California Alliance for Health (the Alliance) and support from the Department of Health Care Services (DHCS), we are proud to offer this innovative service to select Medi-Cal members in Merced County and beyond.

About the program

Enhanced Care Management is a statewide Medi-Cal benefit available to select members with complex needs. Enrolled members receive comprehensive care management from a single lead care manager who coordinates all their health and health-related care, including physical, mental health, and dental care, as well as social services.

This program is focused on providing holistic care beyond traditional medical services, addressing both clinical and non-clinical factors that impact health outcomes.

Enrollment in ECM is offered at no cost to eligible Medi-Cal members, ensuring that everyone has access to the support they need.

The Care Coordination Team can meet ECM members where they are. This may mean on the street, in a shelter, in their provider’s office, or at home. Through this program, staff will connect members to community support services that can help patients get access to nutritious food, safe housing, public subsidies, or whatever services are required to complement the care they receive by Golden Valley Health Centers. As part of a Population Health Management program within CalAIM, ECM focuses on special patient populations and helps select Medi-Cal patients avoid the challenge of engaging with multiple delivery systems for care and services.

“Last week, we contacted the parents of a 13-year-old child that lives with four of his other family members in Merced. Given the size of the family, and the fact that they are considered to be low income, the child and his family were not receiving a number of benefits that are available to them locally in Merced. After a conversation with one of our Lead Care Managers, we were able to determine that he was eligible to be enrolled in Enhanced Care Management. In about 10 minutes we were able to refer his parents to a local food pantry, and put them in touch with an organization that helps secure housing and financial assistance. Our staff also determined that the household qualified for assistance with their utility bills. Additionally, we learned that his grandmother lives with him and often has difficulty getting transportation to her weekly doctor visit at one of our clinics. We were able to determine that she too, was eligible for Enhanced Care Management services and referred her to a local group that will pick her up while her daughter is working.” -GVHC Staff
