The Administrative Receptionist provides administrative support to the Director of PACE and leadership team for PACE.  Under the direct supervision of the PACE Center Manager, the Administrative Receptionist duties include general clerical, receptionist and project based work, answers all administration calls, type documents, and greets visitors to the PACE Center.  The Administrative Receptionist should project a professional and cordial image through personal and phone interaction.

Schedule is Monday – Friday, working from 8:00am – 5:00pm.

Provides individualized diabetes self-management education and training to individuals and groups including, but not limited to, those patients enrolled in the Sweet Success program according to the scope and standards of practice for Certified Diabetes Educators.

The Data Analyst provides data management and analysis services to the Central Valley PACE Leadership team. Analyst helps determine data collection points and develops and shares various reports needed within PACE and outside agencies; reviews data flow and process, develops and automates or streamlines various reports; creates a centralized repository for information needs.

Under the supervision of the Transportation Coordinator, the Driver is responsible for the direct transportation of physically limited passengers to and from their destinations.  The Driver position requires constant interaction with caregivers and community agencies.

Responsible for providing support and advocacy services to patients of the Central Valley PACE program, including participant screening, assessment, planning, coordination of patient services and care, counseling and referral.

The Central Valley PACE Health Plan Specialist supports the team through data analysis, interpretation, reporting, and modeling health plan operations. Supports aspects of monthly and ad hoc reporting and assists in the preparation of pro formas based on historical, comparative, and projection data. Assists to ensure health plan operations, processes, procedures, policies, and compliance efforts are in compliance with regulatory agencies.

Schedule is Monday – Friday, 8:00am – 5:00pm.

The Intake Specialist oversees the intake and enrollment process of new Program of All-Inclusive Care for the Elderly (PACE) participants. Responsible for educating new potential participants and their caregivers on the benefits of Central Valley PACE Program. Will work independently to gather information, set appointments for assessments and document skilled needs for the incoming cases. Interacts with family caregivers and participants, providing a tour through PACE center, gathering crucial medical and social needs for new participant for enrollment processes.

Schedule is Monday – Friday, working from 8:00am – 5:00pm.

The Outreach Specialist plans, organizes, and directs all outreach and marketing efforts.  Involved in the intake and enrollment, and coordination with the interdisciplinary team as required to facilitate new PACE participant enrollment.

The PACE Center Coordinator Registered Nurse has the responsibility for providing direct and indirect nursing care to assigned patients.  Manages complex clinical situations. Works under the supervision of the Center Manager for Administrative purposes, and under the Medical Director for clinical purposes.

The Physical Therapy Aide (PTA) supports the mission, vision, and values of Golden Valley Health Centers Central Valley PACE participants. Will be responsible for the implementation of a physical therapy program for PACE patients, helping each achieve independence and safe living abilities.

The Quality Assurance Manager – RN develops, organizes and monitors a Quality Assessment and Performance Improvement Plan (QAPI) for the PACE program. The Quality Assurance Manager is responsible for developing the QAPI annual plan, and guiding the implementation of the plan. Ensuring that data is collected from all appropriate sources; and the data is examined and the results are shared with all stakeholders. Oversees the process to evaluate and resolve medical and non-medical grievances and appeals by participants, their family members, or representatives. Acts as the HIPAA Privacy Compliance Officer for PACE and will ensure the security of Medical Records in conjunction with the Center Director. The Compliance Officer for Medicare Part D Fraud, Waste and Abuse (FWA), and will ensure compliance with all CMS and State required activities. Responsible for oversight of the Contracts and Credentialing Coordinator along with the Compliance of this department.

The Recreation Therapist is responsible for the coordination and planning of the therapeutic and recreational activities for the PACE participants of the program.

The Registered Dietitian (RD) supports the mission, vision, and values of Golden Valley Health Centers by providing nutrition care and education to Central Valley PACE participants.  Evaluate the participant’s nutritional status, develop and implement nutritional care plans, assess nutrient requirements, recommend nutritional prescriptions, evaluate participant tolerance and effectiveness of nutritional care, and communicate nutritional data and information to the physicians and other health professionals.

Schedule is Monday – Friday, working from 8:00am – 5:00pm.
