As we grow older, we have increasing needs for medical care. Older patients tend to have more medical conditions, take more medications and are often being treated by more than one clinician. At Golden Valley Health Centers we encourage our patients to manage their own health with the help of an interdisciplinary health care team. This team includes the doctor, nurse, pharmacist, health educators counselors, nutritionist, social worker and others to work together with each patient to create a plan of care.
