Living with diabetes can be challenging, but you do not have to manage it alone. Our Diabetes Self-Management and Education (DSMES) program provides the knowledge, skills, and support you need to successfully manage diabetes and improve your overall health.
Our team works with you to develop practical strategies for healthy eating, physical activity, medication management, blood sugar monitoring, and reducing the risk of complications.
What is DSMES?
DSMES is an evidence-based service designed to help individuals with diabetes:
- Understand their diabetes diagnosis
- Learn how to monitor blood glucose levels
- Develop healthy eating habits
- Increase physical activity safely
- Manage medications effectively
- Prevent or delay diabetes-related complications
- Build confidence in daily self-care

- People newly diagnosed with Type 1 or Type 2 diabetes
