Golden Valley Health Centers (GVHC) is partnering with Merced United to ensure that your child’s health and fitness are evaluated, promoting a healthy start to their sporting activities.
Their Medi-Cal Play Program supports participants in both Merced United Recreational Soccer program for youth and Merced United League for adults, reimbursing registration fees upon the completion of a sports physical. Enroll yourself or your child in our Merced United soccer programs, and once you/they complete a physical with one of our partnered health centers or other health care provider, they will reimburse the registration fees. It is important to note that your immigration status does not matter and is not affected when applying for Medi-Cal services.
How to Participate (Youth and Adults):
-
Register: Enroll yourself or your child in Merced United’s Recreational Soccer or League programs.
-
Complete a Physical: Schedule and complete a sports physical examination with one of our health partners or Health care provider that provides services to Medi-Cal members.
-
Submit Proof: Provide proof of Medi-Cal membership and the completed physical (corner screenshot of After Visit Summary or equivalent, do not share personal information) to Merced United Program Coordinator (christopher@mercedunited.com) to receive the registration fee reimbursement.
-
Receive a Refund: The account you used to register for either program will be refunded by the end of the Season.
